Our dedicated staff of IIC clinicians provide youth, adolescents and their families with individualized, multi-systemic, goal-orientated, and strength-based clinical intervention that addresses the emotional and behavioral needs of youth and adolescents.

Resources for the Disability Community

Many services for individuals with disabilities are offered by the State and various other organizations. 


Here, families can find information on available food, income, or employment assistance programs; caregivers can find information on support programs and services to assist them in caring for their loved ones; and individuals with disabilities can find information on various services including in-home supports, vocational rehabilitation, personal care assistance, and education.

Who We Are


Eleos Family Services is a DDD approved provider of Community Care Program (CCP) and Supports Program (SP) assisting adults with IDD and their families in their communities. 

Eleos is an approved provider of Intensive In-Community Services (IIC) and Behavioral Assistance. These services are offered through the Children's System of Care (CSOC), under the New Jersey Department of Children and Families’ (DCF).

Our mission is to provide enhanced services to Children, Adolescents, Adults with IDD and their families, who are in need of Support Services, Behavioral/Mental health services by serving, facilitating, educating with a collaborating care approach.

Our vision is to provide compassionate & person-centered care to Children & Adolescents, and People with IDD.  We are committed to supporting people to receive their requisite opportunities, reach their goals and realize their potential as valued, independent, meaningful contributors within their communities and to achieve a better quality of life.

Our staff members are certified and/or licensed professionals with many years of experience. We are a team of caring people and dedicated professionals who are passionate about promoting personal independence and community integration.

Our Youth


Our Youth and Families receive individualized, multi-systemic, goal-orientated, and strength-based clinical intervention that addresses the emotional and behavioral needs of youth with moderate to high needs. Behavioral Assistants address behavioral needs by developing skills with families aiming to increase desired behaviors and implement healthy coping strategies.

Youth, Adolescent and Individual rights and responsibilities are of upmost importance as each person, is just that "a person", a human being created with value, dignity and individuality that we believe is to be honored. In addition, rights support strong communities, empower people, and create a society that values equality and liberty. Eleos aims to uphold, honor and protect the rights & responsibilities of the Youth, Adolescents, Individuals and families that we provide supportive care services to. It is the policy of Eleos to that our support staff embody and exhibit our Guiding Principles and Code of Conduct with regards to how the persons we support are both viewed, valued and treated.

Participant Rights & Responsibilities

People with IDD have the same human rights as all people and are entitled to the same benefits and legal protection of their civil rights. They are entitled to exercise their rights and to have their human rights and civil rights respected. Persons with disabilities have the same rights as all people to non-discrimination, access, equality of opportunity, inclusion and full participation in society

Children's Rights & Responsibilities

Child/Adolescent Rights & Responsibilities were established with the expectation that observance of these rights will contribute to more effective Child/Adolescent care and greater satisfaction for the Child/Adolescent, family, provider, and agency. Child/Adolescents shall have the following rights inclusive of any age, race, ethnicity, national origin, color, gender identity/expression, sexual orientation, religion, culture, ability/disability, personal values or belief systems.

Complaint & Grievance

In accordance with DDD, DHS, CSOC and our Grievance Policy, Employee Handbook, Code of Conduct and Youth, Adolescent and Individual Rights, its is the expectation that staff act in an ethical manner, misconduct is prohibited. The below form is available as a method for Youth, Adolescents, Individuals, Families/Guardians to voice their concerns, complaints and grievances. It is the right of all persons and their families who we support to submit a complaint or grievance when they feel it is needed either for themselves or for their loved one. It is the policy of Eleos to process all grievances in a timely, impartial, and confidential manner.


Eleos will not, under any circumstances blame the reporter, conceal a report or discourage a staff, Youth, Adolescent, Individual or their family/guardians from reporting discrimination. If a staff member behaves that way, please send an email to their own manager, the Human Resources Director (HRD), Compliance Manager or Executive Director explaining the situation. 


All reports and complaints are treated with discretion, ensuring minimal disruption during investigations and preserving the reporter's anonymity, when possible.

 

If you are aware or suspect employee misconduct of any kind against yourself or Youth, Adolescent, Individual or their families/guardians, please report it as soon as possible.


Examples of misconduct may included but is not limited to: 

  • Medicaid fraud, waste or abuse
  • Youth, Adolescent or Individual's Rights violations or infringement
  • Bullying
  • Discrimination
  • Harassment
  • Retaliation
  • Inappropriate Workplace Behaviors


We aware that it is often hard to come forward about these issues, but we need your help to maintain a strong, fair and safe workplace for you and your coworkers. 


Please do not let these fears deter you. Our company will do everything possible to address violations and while supporting staff members who report incidents. We need to know what’s going on so we can act on it. By raising your voice on this issue, you help to support Individuals and protect our company culture.


Our company strictly prohibits any and all forms of retaliation against staff or Individuals, families or guardians who report violations in accordance with our Complaint & Anti-Retaliation policy, which describes our provisions towards staff, Individuals, families or guardians who file reports for harmful, discriminatory or unethical behaviors. Whether accusations are true or false, our company wants to prevent victimization and other retaliatory behavior towards a reporter. We believe it is important that no one is afraid to speak up about any issues or concerns.

Adults with IDD

We provide a wide range of services to promote and protect the human rights of people with intellectual and developmental disabilities and actively support their full inclusion and participation in the community throughout their lifetimes.


Children & Adolescent

We offer various treatment services for children and adolescents who have been struggling with all types of mental and behavioral health disorders, including substance use. Children and adolescents can receive comprehensive, personalized care with Eleos.

Community Based Support & Individual Supports


Services that provide direct support and assistance for participants, with or without the caregiver present, in or out of the participant's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Individualized Service Plan (ISP).

Supported Employment


Services needed to help a participant obtain and maintain an individual job in competitive or customized employment, or self employment, in an integrated work setting in the general workforce for which an individual is compensated at or above the minimum wage, but not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities.

Intensive In-Community Services

Children & Adolescent


Intensive In-Community Service (IIC) is an incisive, goal oriented, and needs based clinical intervention that addresses emotional and behavioral challenges of youth with moderate to high needs.  We will work in collaboration with your family and the care team to stabilize and support youth to address presenting behaviors that will result in the youth and her/his family/care giver/guardian gaining insight and improving function at home and in the community.
